Ownership Change

If your pharmacy gets bought out or you intend to sell, we will need to update all the ownership information in our system. 

This entails you filling out the Plenty RX application with all of the new owner's information. Please note that if the DEA license is staying the same, we will need a copy of the power of attorney showing the new person/owner appointment.
